To be sent to the head of DT at the KS3 provider when complete.
A questionnaire for the teacher responsible for managing the transition of KS2 ? 3, that investigates the general transition policy of the school. To be sent to their equivalent at the KS 3 provider when complete.
An individual assessment form for class teachers, containing space to record pupil names, attainment levels at KS2, and expected attainment levels at KS3. To be sent to the relevant DT teachers at the KS3 provider when complete. This step may not be necessary depending on a schools' report-issuing policy, but in my experience, KS2 reports (at year 6) tend to contain general comments on non-core subjects rather than details of actual attainment levels.
Part 2: To be sent to the KS3 Provider, containing:
A form for the head of DT, that collates information about the work that will be undertaken at KS 3. To be compared with the information obtained from KS2 providers to identify areas in which there is a lack of continuity between levels.
A questionnaire for the teacher responsible for managing the transition of KS2 ? 3, that investigates the general transition policy of the school. To be sent to their equivalent at the KS 2 provider when complete.
A questionnaire to be sent to individual DT teachers asking for information about their plans for monitoring pupil progress, particularly with reference to expected and actual attainment during the first year of KS3. To be included in Part 3.
Part 3: The Summary document that identifies:
Areas of good and poor continuity in the teaching of DT at both levels, in the form of a written summary of the information obtained from parts 1 and 2.
Potential routes for improving areas of poor continuity.
The predicted grades for pupils making the transition during the current academic year.
There should also be space for further work, which will include comparison of these predicted grades with actual grades obtained at KS3.
3.3 Conduct of Research
Note that summaries of the expected and actual grades of named pupils are confidential material: if the research is to involve or be communicated to people outside of the schools in question, names must be removed and numerical or other generic identifiers used.

Subject to the requirements of individual schools, it is suggested that the research pack be sent out at the beginning of Year 6 for the pupils involved, with the aim that the summary document be available at the end of the first term. This allows staff at both schools to use the information obtained to manage the transition period (e.g. the summer term) effectively based on the information obtained. Having obtained the consent of the head-teacher, parts 1 and 2 of the research pack would be issued and completed by school staff.
